Resumo: dc.description.abstractIntroduction: Chronic pain affects a significant portion of Brazilian society, with physical trauma, such as hospitalizations resulting from car accidents, being one of the main early factors in its etiology. In this context, neuropathic pain presents itself as one of the most important and disabling types of this pathology, making it challenging for the multidisciplinary team, requiring different approaches for the successful treatment of patients. Objective: To report a case of peripheral nerve block as a surgical intervention in the treatment of neuropathic pain for patients and with low adherence to the proposed therapeutic project. Methodology: The data were made available on a consented and voluntary basis by the medical team. Information was collected using the semi-structured interview technique with the medical team. Case description: Female patient, 55 years old, suffered a car accident with trauma to the hip and right lumbar region. Therefore, on the day of the accident, she underwent exploratory laparotomy with right nephrectomy, osteosynthesis of the right femur and right lunate. However, she developed pain in the left lumbar region, which did not improve with the widely used medications prescribed, in addition to having low adherence to the proposed therapeutic regimen, due to her difficult socioeconomic situation. Therefore, we opted for peripheral nerve blocks, in several sessions, which were performed on an outpatient basis. Conclusion: the choice of peripheral nerve blocks in the lumbar region as a treatment proved to be effective and reduced the symptoms of neuropathic pain by up to 60%, which remained with an intensity between 3 and 4 on the visual analogue scale and with no pain to touch of the scar.pt_BR
