Data were obtained through semi-structured interviews with the interviewees, in addition to systematic observation and document analysis, and the transcribed information was subjected to thematic-categorical content analysis. Results: from the analysis of the participants' discourse, the following categories and subcategories emerged: Perception of home care; Admission process and eligibility criteria; Disconnection of the patient from the Home Care Services. Conclusion:From the perceptions and experiences of the managers and health professionals of the Home Care Services, it is emphasized that each of the services has its individualized implementation according to the care profile of the hospital unit to which it is linked, and with the composition of the multidisciplinary team according to the extension of service.
