Resumo: dc.description.abstractIntroduction: Cecal volvulus corresponds to 1% of causes of obstructive acute abdomen, resulting from torsion of the intestinal cecum over its own mesentery. Clinical presentation is variable, from intermittent, self-limiting abdominal pain to acute abdominal pain associated with intestinal ischemia and sepsis. Methodology: Female, 78 years old, abdominal pain and distension, cessation of elimination of flatus and feces, emesis and nausea. Through exploratory laparotomy, cecum volvulus was identified. She underwent ileocolectomy and terminal ileostomy, with good results. Results and discussions: Faced with an obstructive abdomen, imaging tests and colonoscopy are useful to guide the diagnosis. Therapy is influenced by intestinal vitality, covering procedures such as colonoscopic reduction to right colectomy. Conclusions: This report emphasizes the importance of cecal volvulus as a diagnostic hypothesis in obstructive conditions, aiming at an agile diagnosis and subsequent correction, avoiding complications such as intestinal perforation and ischemia.pt_BR
